Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
An unannounced code compliance inspection was initiated on 2/25/2022 and concluded on 3/1/2022. The person in-charge was contacted by telephone and a virtual inspection was conducted. There were 22 children present, ranging in ages from three to five, with five staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and procedures. A total of two children?s records and two staff?s records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22.1-289.031-B-4
Description: Based on interview, the center failed to implement the center's procedure to ensure all children are in compliance with immunization provisions.

Evidence:

1. The administrator stated the center's policy for immunizations is as follows: Upon enrollment or before the first school day parents need to submit the most up to date immunization records of their child. If somebody does not have an immunization record then we ask them to submit the Religious Exemption Form.
2. The record for child 2 was reviewed. The first date of attendance was documented as 1/31/22. The immunization record was dated 2/25/22.
3. The administrator stated "The parents never gave them the copy when registering. I reminded them a few times but then we were on quarantine for 10 days and got back to school on February 23."
She brought it on the 25th.

Plan of Correction: In the future no child will be allowed to start attending until the center receives the immunization records.

Standard #: 8VAC20-770-40-D-2
Description: Based on record review and interview, the center failed to have a completed central registry (CPS) record check by the end of the 30th day of employment.

Evidence:

1. The record for staff 1 documents the hire date as 8/18/21. The copy of the CPS in the file does not have the findings documented. The copy of the check for the CPS is dated 8/17/21.
2. The administrator stated the CPS document was mailed but the findings were never received. They never followed up to find out what happened to the document.

Plan of Correction: The administrator called on 3/1/22 to follow up and the results could not be found. Another request will be mailed as soon as the document can be completed. If the results are not received within 30 days the administrator will follow up. The results will be sent to the licensing inspector when received.
