This Licensing Inspector arrived at 11:40 AM and departed at 12:35 PM to conduct a subsidy health and safety inspection (SHSI) in conjunction with a religiously exempt child day center code compliance visit a subsidy health and safety inspection (SHSI). At the time of the inspection, there 12 children (ages infant through four years) in care with four staff. Children were observed napping, being fed, and interacting with staff. The sample size was five staff records and five children's records.

Standard #: 8VAC20-790-560-A Description: Based on record review and interview it was determined the center did not ensure that all staff were evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.
