A complaint regarding the playground structure was conducted on 6/30/22.
This complaint is founded and the center was advised that the playground structure must be closed off and not used by the children. The space around the structure can still be used and the church has other play areas that the children can use.
Discussion of adding an infant toddler room was also part of this inspection, LI and director discussed zoning requirements and what changes needed to be made to the physical plant.
The ongoing compliance with the standards is the responsibility of the licensee.

Standard #: 22.1-289.031-B-5 Complaint related: Yes Description: Based on the review of the playground equipment it was determined that the center failed to ensure that the climbing structure was free of obvious injury hazards.
Evidence: The wooden structure on the playground has several protruding rusted screws, broken or rotten boards and a slide with a 6 inch split in the bottom of the slide causing a pinching or cutting hazard.Plan of Correction: We will not use the playground structure and will inform all classes to stay off the structure. We have other spaces we can use for outdoor play and will plan on getting a new structure to replace this one.
