Today an unannounced Monitoring Inspection took place between the hours of 12:15pm and 1:30pm. There was a total of 17 children with 5 staff within the supervision guidelines. The children were observed eating lunch, playing games, and other organized activities. A complete inspection of the physical plant, staff records, and observations were made during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Standard #: 22.1-289.035-B-4 Description: Based on a review of staff records, 1 out of 4 staff did not have the results of a search of the child abuse and neglect registry of equivalent for a state in which the individual has resided in the preceding five years.
Evidence:
1. Staff #4 (doh: 12/1/2021) did not have the results of an out-of-state background check for state #1 in which the employee has resided in the preceding five years.Plan of Correction: We will follow-up on this since we sent it and and we will document the follow-up.
Standard #: 8VAC20-770-40-D-2 Description: Based on a review of staff records, 3 out of 4 staff did not have the results of a central registry background check within 30 days of employment.
Evidence:
1. Staff #1's (doh: 11/22/2021) central registry results were dated 1/8/2022.
2. Staff #3's (doh: 12/1/2021) central registry results were dated 1/8/2022.
3. Staff #4's (doh: 12/1/2021) central registry results were dated 1/8/2022.Plan of Correction: We will follow-up if central registries are not received within 30 days.
