An unannounced monitoring inspection was conducted from 10:15 am - 11:30 am. There were five children in care with the provider ages, 9 months, 12 months, 2 years, 3 years, and 3 years for a total of 14 points. The children were observed in free play. Lunch served today is corn dog nuggets, green beans, orange slices, bread, and milk. Five children's records were reviewed. The provider record was reviewed and in compliance. The most recent fire drill was conducted 05/01/2015. The provider does not administer medications in the family day home. MAT (Medication Administration Training) is required to administer medication to children in the family day home. Standard #: 22VAC40-111-50-A Description: Based on review of the family day home and interview with the provider it was determined that children's attendance is not recorded daily. Evidence: The provider informed the LI (Licensing Inspector) that a record of attendance is not kept at the family day home. Plan of Correction: The provider will record attendance daily moving forward.
Standard #: 22VAC40-111-60-B Description: Based on review of 5 children's records it was determined that not all records were complete. Evidence: Child B with a start date of 8/22/2014 did not have the address for 2 required emergency contacts. Plan of Correction: The provider will request the emergency contact addresses for child B.
Standard #: 22VAC40-111-520-C Description: Based on review of the family day home it was determined that not all cribs met the U.S. Consumer Product Safety Commission. Evidence: The crib utilized for Child A had a manufacture date of March 2009 and does not meet the U.S. Consumer Product Safety Commission standards. Plan of Correction: The provider will order a crib that meets U.S. Consumer Product Safety Commission standards.
Standard #: 22VAC40-111-520-E Description: Based on review of the family day home it was determined that crib bumper pads were not prohibited. Evidence: The crib utilized for Child A was lined with bumper pads. Plan of Correction: The provider will remove the bumper pads.
Standard #: 22VAC40-111-540-B Description: Based on review of the family day home it was determined that walkers were not prohibited in the family day home. Evidence: Child A was observed in a infant walker. Plan of Correction: The provider will dismantle the infant walker and discard.
