Discussion was held on the topics of current enrollment, current staffing, and zoning.

The following inspection took place between the hours of 11 am through 1 pm. Today, I met with the Provider on site at her Family Day Home (FDH) located in Centreville, Virginia. At the start of this unannounced renewal inspection there were a total of 10 children (see violation notice) (plus 3 in residence) (23 points) with 2 caregivers. A total of 3 children?s files and 6 staff/household member?s files were reviewed. The children were engaged in the following: outdoor play, bathroom breaks, hand washing, diaper changing, games, story time, lunch, nap time, and other organized activities. Description: Based on review, interview, and observation, the current capacity on the date of inspection exceeds the approved capacity established on the Contact With Local Zoning Administrator form. Evidence - 1. The Family Day Home (FDH) has relocated to a new address. There former home had a capacity of 12. 2. The new home is permitted by-right with a maximum of 7 children at any one time, exclusive of the Provider's own children. 3. The Provider stated (during an interview conducted on the date of inspection) that at this time no special permit has been filled or approved. Once a special permit by the Board of Zoning Appeals (BZA) has been approved the number of children may be increased up to a maximum of 12. 4. On the date of inspection (07/07/2015), there were 10 children in care, plus 3 children in residence.

Plan of Correction: An application will be filled with the BZA.

Description: Based on observation, play equipment was positioned over a hard surface. Evidence - 1. On the date of inspection (07/07/2015), a slide and a piece of climbing equipment with a slide was observed over a hard surface consisting of concrete and stone. 2. The Provider confirmed this observation on the date of inspection.

Plan of Correction: The items will be moved on to the grassy area.
