Comments:
The licensing inspector conducted an unannounced monitoring inspection on Tuesday, July 7, 2015 from 11:15am to 1:30pm. The children were observed playing with the toys in the home upon the inspector?s arrival, and they were later observed playing ring-around-the rosie with the caregiver. Today, there were five children, totaling 13 points, present with one caregiver. Two children?s records and one caregivers? record was reviewed during the inspection. Last emergency drill: 6/4/2015 Emergency Plan Reviewed: 11/13/2014 The violations from the previous inspection in November of 2014 were checked for corrections. Standard #: 22VAC40-111-60-A
Description: Based on a review of children's records and caregiver interview on 07/07/2015 the provider did not ensure that each enrolled child had a record. EVIDENCE: Child #1, who, according to Caregiver #1, started on 6/29/2015, did not have an enrollment form. The provider shall maintain an up-to-date record at the family day home for each enrolled child.

Plan of Correction: Will have missing enrollment form by the end of the day today, 7/7/2015.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records on 07/07/2015, the provider did not ensure that each child's record reviewed contained the required information. EVIDENCE: The record of Child #2 did not contain the house number and street name of the 2 emergency contacts listed. Each child's record shall contain the name address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached.

Plan of Correction: Will get the child's father to complete the missing information today, 7/7/2015, at pick up.

Standard #: 22VAC40-111-540-B
Description: Based on observation on 7/7/2015, the provider did not ensure that infant walkers were not used. EVIDENCE: Upon the inspector's arrival, Child #1 was observed in an infant walker. Infant walkers shall not be used.

Plan of Correction: Corrected on site. Child removed from walker. Walker will not be used anymore.
