A self report inspection was initiated on 7/15/2022 and concluded on 8/26/2022 regarding an allegation relating to supervision of children. Interviews were conducted with staff and videos were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.031-B-2 Description: Based on interviews and review of footage from the security cameras, the center failed to ensure that the center implemented procedures for appropriate supervision of all children in care.
Evidence:
1. The center?s staff handbook states under ?Playground Safety? that: ?The biggest factor in maintaining a safe environment for the children is close and constant supervision.? Staff #1 was approximately 20 feet away from and constantly looking at her phone during the one minute that two children (ages 18 & 20 months) were fighting over a riding toy.
2. The center?s staff handbook in the ?Telephone Calls? section states, ?No texting or other cell phone activities will be allowed during work hours.? Staff #1 was observed on her phone on 7/15/2022 while on the playground supervising four toddlers. Staff #1 was observed using her cell phone approximately eight times totaling approximately 10 minutes during the 30 minute playground supervision of four children ages 15 months -20 monthsPlan of Correction: The director terminated staff #1's employment as soon as she was made aware of the complaint and verified what happened.
