Today an unannounced renewal study was conducted beginning at 10:40am. There were 5 children (14 points) present with the Provider. Children were observed reading books, playing with puzzles, and eating lunch. Record keeping for adults, children and emergency procedures/drills has improved. There are currently no medications for children on-site at this time. The Provider has already attended 8 of 16 annual required training hours required by June, 2016. Standard #: 22VAC40-111-60-B Description: Based on record review, one child's record (#7) is missing complete required documentation in their record. Evidence: Child #7 has a start date of 9/22/14. There is no documentation that a proof of identity was viewed and verified by the Provider. There is also no documentation of a physical examination obtained within 30 days of the start date for child #7. Plan of Correction: I will ask the parents to provide me with this documentation again. If they do not provide proof of identity I will report it to local authorities. I thought they gave me the physical but I think I got it mixed up with the two pages of immunization records.
