Thank you for your time and assistance with the unannounced monitoring inspection conducted today, July 20, 2015. The on-site visit was from 11: 10 a.m. to 1:10 p.m. On this date, at the time of arrival, there were twelve (12) children in the care of the provider and one assistant for a total of twenty-four (24) points. There were four (4) children's records reviewed. The records for the provider and two assistants were reviewed and updated. Areas of non-compliance may be found on the violation notice. Risk assessment scores were discussed. No medication is being administered at this time. Some of the items observed and reviewed during the visit included: evacuation drills, first aid supplies, TV time, lunch prep, hand washing and bathroom procedures, lunch, free choice activities and movement. Interactions between the provider, assistants and the children were positive and the children seem comfortable with the daily routine and schedule. Standard #: 22VAC40-111-100-A Description: Child C did not have documentation of obtaining a physical within 30 days of attendance. First day of attendance was documented as 12/15/2014. Evidence: Based on record review and conversation with the provider it was confirmed Child C did not have documentation of obtaining a physical within 30 days of attendance. Plan of Correction: The provider will request the parent provide documentation of a physical examination for Child C. In the future, all children will provide documentation of a physical within 30 days of attendance.
Standard #: 22VAC40-111-60-B Description: Child A did not have documentation of parent's annual review of emergency contact information. Last documented review was 03/14/2014. Evidence: Based on record review and conversation with the provider it was confirmed Child A did not have documentation of annual review of emergency contact information. Plan of Correction: The provider will request the parent review and update Child A's emergency contact information. In the future, annual updated will be completed.
Standard #: 22VAC40-111-90-A Description: Child C did not have documentation of obtaining immunizations. First date of attendance was documented as 12/05/2014. Evidence: Based on record review and conversation with the provider it was determined Child C did not have documentation of obtaining immunizations. Plan of Correction: The provider will request the parent provide documentation of Child C receiving immunizations. In the future, all children will provide documentation of immunizations by the first date of attendance.
