An unannounced Subsidy Health and Safety inspection was conducted today from approximately 9:45am-11:10am. There were no children in care during inspection. The home, emergency supplies and plans were inspected. Children?s records, vendor records and household member documentation were also reviewed. There were no medications on site today. Description: Based on review of child information it was determined that all required information was not maintained. Evidence: During inspection Child1 and Child2 were missing emergency contacts and permissions for transportation. Child3 was missing permissions for transportation. Child4 was missing parent's workplace and phone number and permissions for transportation.

Plan of Correction: I will get the information from parents and keep it
