Technical Assistance:
Please review Standards 500- Outdoor Play Area and Equipment. The resilient surfacing is deep and limits the height of the highest platform on outdoor equipment from exceeding four feet but the Provider needs to maintain that ratio. Door leading to backyard (from garage) was observed and it's current condition may require replacement. Discussed what references are accepted and how they should be recorded.

An unannounced inspection was conducted today from 100-215. There were 9 children with two caregivers for a total of 21 points. There were also family members staying with the Provider and the Provider had a record for the visiting infant. The Provider's records were reviewed and this includes four children's records. The Provider no longer administers medication and the children's records include a statement that medication is not given. The Provider has another assistant who helps in the afternoon and her records were maintained. Please call with questions 703-479-4679

Standard #: 22VAC40-111-280
Description: Based on observation and the ages of children in care, the Provider does not have changing area cleaning items stored in an area away from children's access. Evidence: 1. Changing table is located close to small set of stairs that leads into the children's main play area. The shelf above the changing table is also by the open stairway and this shelf contains cleaning spray, disinfecting wipes, and air freshener spray. 2. There is a school age child enrolled in the program and the Inspector observed him walk down the stairs and noted the cleaning items are within his arm's reach.

Plan of Correction: Moved during the visit.

Standard #: 22VAC40-111-530-F
Description: Based on observation, the Provider allowed an infant to rest in the crib that had a soft bedding comforter as a blanket. Evidence: 1. Inspector walked into the sleeping room and noted an infant that was laying on the back in a crib. There was a soft filled comforter covering this baby.

Plan of Correction: Removed during the visit.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on record review, the Provider had not obtained a search of central registry for staff #2 before three years since the date of the last most recent central registry finding report. Evidence: 1. Assistant #2's record was reviewed and the Inspector found a most recent central registry search had been returned to the Provider and, highlighted in blue, it showed the social security number was not recorded. The Provider thought it had been completed but the Inspector showed the back page indicated the check was not conducted because it was missing info. 2. The Assistant's original CPS check was conducted in July, 2012.

Plan of Correction: I will send back out.
