Discussion was held on the topics of current enrollment and current staffing.
The following inspection took place between the hours of 1 pm through 3 pm. Today, I met with the Provider on site at her Family Day Home (FDH) located near the town of Aldie, Virginia. At the start of this unannounced monitoring inspection there were a total of 8 children (18 points) with 2 caregivers. A total of 3 new children?s files and 4 staff/household member?s files were reviewed. The children were engaged in the following: nap time, snack time, outside play, and other organized activities. Standard #: 22VAC40-111-60-B Description: Based on review, 1 of 3 new children's files reviewed indicated that not all of the required information was contained within each child's record. Evidence - 1. On the date of inspection (09/08/2015), the policy number of Child #2's medical insurance. 2. While the name of the policy was listed, the number was not included. Plan of Correction: The policy number will be obtained from the Parents of Child #2.
Standard #: 22VAC40-191-40-D-1-c Description: Based on review, 1 of 4 staff/household members' files indicated that it has been more than three years since the dates of the last sworn statement or affirmation (SDS), most recent central registry finding (CPS), and most recent criminal history record check report (CRC). Evidence - 1. On the date of inspection (09/08/2015), the following documents were older than 3 years for Member #2: SDS (dated - 07/01/2012), CPS (dated - 07/15/2012), and the CRC (dated - 06/25/2012). 2. These three documents were viewed within the folder for Member #2 on the date of inspection. Plan of Correction: New documentation will be obtained for Member #2.
