Standard #: 22VAC40-111-840-B
Description: Based on information from the licensee, injuries are not recorded in the children's records. Evidence - The licensee stated that injuries are reported to parents by text and are not recorded in writing in the children's files.

Plan of Correction: In the future will be reported via email and/or writing.

Standard #: 22VAC40-111-50-C
Description: Based on a review of information provided to a parent, the licensee did not keep all information in one child's record confidential. Evidence - The notification, by text message to a parent gave another parent's child's name in an injury report.

Plan of Correction: It was of common knowledge who was inflicting the bites & his parent were the one who extended a public apology to all the other parents for the son's behavior!

Standard #: 22VAC40-111-210-A
Description: Based on a review of staff records and information from the licensee, the staff and licensee have not completed the annual training hours. Evidence - The licensee has of 16 annually required training hours. The assistant has 10 of 16 required training hours.

Plan of Correction: In the future will comply w/ this requirement.

Standard #: 22VAC40-111-530-F
Description: Based on observation, the licensee did not prevent the use of a pillow in a crib. Evidence - There was a boppy pillow in a crib with one infant.

Plan of Correction: For the record it was not a Boppy Pillow, it was a neck (roll) pillow the almos [sic] 1 yr old likes to play with. In the future cribs will be kept bare.

Standard #: 63.2-1701-F
Description: Based on observation, the licensee did not post the violation notice in a conspicuous place. Evidence - The violation notice was posed behind the inspection summary in a clear sleeve that was tacked to the wall at the top so that the violation notice could not be seen without removing from the wall.

Plan of Correction: In future will try my best to display on [sic] plain view of Inspector
