The application can not be processed until all of the required background checks have been returned with satisfactory findings. Please contact me as soon as the search for the central registry checks are returned.

An announced initial study was conducted with provider. There were no children in care today, so not all standards could be reviewed. Three household files were reviewed. The basement is well furnished for day care with a range of appropriate activities and equipment. Provider will need to renew first aid and CPR certification before 8/19/2015. Standard #: 22VAC40-111-760-A-1
Description: Based on review of the first aid kit, the provider failed to have all of the required emergency supplies in the kit. Evidence: there were no triangular bandages in the first aid kit.

Plan of Correction: Provider will purchase or will make triangular bandages for the first aid kit.

Standard #: 22VAC40-191-40-C-1-a
Description: Based on review of three household files and provider interview, the provider failed to obtain a search of the central registry as required for all household members 14 years and older. Evidence: There was no search of the central registry for Household Members 1 and 2.

Plan of Correction: Provider will send the request for the search of the central registry for the two household members.
