54.1-3408 Provider must be MAT certified to administer prescription medication.

Licensing Inspector Anderson conducted an unannounced monitor inspection was conducted on 9/17/2015. Upon arrival at 10:35 am there were five (5) children ages 22 months to 3 years old and the Provider present. Children present totaled eleven (11) points with one (1) caregiver. LI Anderson reviewed records for recently enrolled children & records for the Provider. No changes in staff or household members since the previous inspection. Daily program activities include, structured activities, meals/snacks, self initiated activities, outdoor play and afternoon nap. Provider continues to be MAT certified however there were no children in care currently receiving medication during the FDH hours of operation. Violations found during the previous inspection were reviewed and have all been corrected. Risk Assessment and areas of non-compliance were reviewed with the Provider on 9/17/2015.

Description: Based on inspection, the Licensee failed to have all required first aid supplies. EVIDENCE: The designated first aid kit did not have scissors, tweezers, surgical/examination gloves, adhesive tape & antiseptic cleaning solution

Plan of Correction: Provider will purchase/re-stock first aid supplies.

Standard #: 22VAC40-111-50-A
Description: Based on inspection & discussion with the Provider, the Licensee failed to keep a written record of children in attendance each day. EVIDENCE: Documentation of daily attendance was not completed from Monday, Sept 14th through Thursday, Sept 17th.

Plan of Correction: Corrected during inspection. Provider will document attendance each day when children arrive & leave.
