54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was conducted today. Eight children were present with the provider and assistant. Total 19 points with two caregivers. The children were observed playing outside at the park, eating lunch and getting ready for rest time. Two children are new since the last inspection. Their records were reviewed today. The provider is MAT certified, but is not administering any medication at this time. If you have any questions, I can be reached at 703 479 4709.
Standard #: 22VAC40-111-770 Description: Based on observation, the battery-operated radio was not working. Plan of Correction: Will obtain a new battery operated radio.
Standard #: 22VAC40-111-100-A Description: Based on record review, the provider did not obtain documentation of a physical exam prior to a child's attendance or within 30 days after the first day of attendance. Findings: At the time of the inspection, the inspector found that there was no documentation of a physical exam for Child #2, who started in care on 7/6/15. Plan of Correction: Will obtain documentation from the parent.
Standard #: 22VAC40-111-90-A Description: Based on record review, the provider did not obtain documentation of immunizations before a child attended the family day home. Findings: At the time of the inspection, the inspector found that there was no documentation of immunizations for Child#1, who started in care on 9/1/15. Plan of Correction: Will obtain documentation from the parent.
Standard #: 22VAC40-111-280 Description: Based on observation, cleaning agents were not stored in areas inaccessible to children. Findings: At the time of the inspection, the inspector observed a container of degreaser cleaner stored in the cabinet under the sink in the downstairs bathroom, that is accessible to the children. Plan of Correction: Cleaner was removed and placed in an area inaccessible to children.
