Technical Assistance:
Background checks (sworn disclosure statement, search of the central registry, and criminal history record) are required to be repeated every three years. TB tests or screenings are due to be repeated every two years.

An unannounced renewal inspection was conducted with provider and two assistants. There were 11 children present, ranging in age from 9 months to 5 years, totalling 28 points. The children enjoyed lunches prepared from home and laid down for rest time. Three children's files and five household/staff files were reviewed - please see Supplemental Summary for missing items. Provider does not administer medication - Medication Administration Training is required to administer medicine. Standard #: 22VAC40-111-180-A
Description: Based on review of staff/household files, the provider failed to have documentation of a repeat tuberculosis test or screening for three household members. Evidence: Household members 1, 2, and 3 did not have current tuberculosis test or screening on file.

Plan of Correction: Household members received a tuberculosis test this past summer and will request the information from the physician.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on review of five staff/household records, the provider failed to have repeat background checks within three years since the date of the most recent background checks. Evidence: Household members 1 and 2 did not have repeat criminal history record checks (expired 2/6/2015) or repeat search of the central registry checks (expired 2/6/2012).

Plan of Correction: Provider completed the paperwork to mail the background checks during today's inspection.
