Discussion was held on the topics of current enrollment, current staffing, and the change of the Administration Medication Policy. The Family Day Home will now only dispense topical creams that are not prescribed by a physician.
The following inspection took place between the hours of 1 pm through 2:30 pm. Today, I met with the Provider on site at her Family Day Home (FDH) located in Leesburg, Virginia. At the start of this unannounced renewal inspection there were a total of 9 children (24 points) with 2 caregivers. A total of 9 children?s files and 6 staff/household member?s files were reviewed. The children were engaged in the following: quiet activities, clean-up from lunch, diaper changing, hand washing, bathroom breaks, movie time, music, nap time, and other organized activities. Standard #: 22VAC40-111-90-C Description: Based on review, 3 of 9 children's files reviewed indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for a child (who was not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years. Evidence - 1. On the date of inspection (10/02/2015), the following children had immunization documentation on file that were older than 6 months in age: Child #5 (immunization page dated - 10/24/2014), Child #6 (immunization page dated - 10/22/2014), and Child #9 (immunization page dated - 03/18/2015). 2. All three listed children were under the age of two years on the date of inspection. Plan of Correction: All applicable immunization pages will be updated and placed on file.
