An unannounced monitoring inspection was conducted today. Observed 6 children (23 points) directly supervised by 2 caregivers. Ratio and supervision were in compliance. A sample of 2 children's records and 2 caregiver records were reviewed. The physical plant, and emergency procedures were also reviewed. No medications were reported to be on site. The children were engaged in various activities, to include: free play, playing with blocks, and infant care. Areas of non-compliance are identified in this report. Standard #: 22VAC40-111-800-B Description: Based upon documentation reviewed, the provider did not review the emergency plan at least annually and update the plan as needed. Evidence: The emergency plan was last reviewed/updated on 09/30/2014. Plan of Correction: I will update the emergency plan today
Standard #: 22VAC40-111-120-B Description: Based upon documentation reviewed, a caregiver record was incomplete. Evidence: One of the two caregiver records reviewed did not contain documentation verifying that a caregiver received the licensing orientation by the end of the first week of assuming job responsibilities (Caregiver #1; Date of hire 04/23/2015). Standard #: 22VAC40-111-50-C Description: Based upon observations made and documentation reviewed, children's records were not kept confidentially. Evidence: Confidential information was posted on the bulletin board and readily viewable by the public and parents. Identifying information about children referenced on the Supplemental Information portion of the March 30, 2015 inspection report was posted. The report states, "Information found on the Supplemental Information page is confidential and this document is not to be posted in the facility". Plan of Correction: I removed the confidential information
Standard #: 22VAC40-111-60-B Description: Based upon documentation reviewed, a child's record was incomplete. Evidence: One of the two children's records reviewed did not contain the address for the second designated person to contact in case of an emergency if the parent cannot be reached (Child A). Plan of Correction: I will ask the parent the information that I need today
Standard #: 22VAC40-111-240-B Description: Based upon observations made, a recalled equipment was in used for an infant. Evidence: During today's inspection, Child B (6 months) was placed inside the Bumbo seat by Caregiver #1. On 08/15/2014, Bumbo International Trust voluntary recalled to add a restraint belt and new warnings sign to the Bumbo Baby Seat. The Bumbo seat in used did not have a restrain belt. Plan of Correction: I will not longer used the Bumbo seat
