Technical Assistance:
Bottles should not be left unrefrigerated for more than 1 hour and may not be warmed more than once due to the possibility that it may harbor bacteria.

We discussed best practices for infant feeding to include having a specific form completed by each infant's parent at enrollment to include what type of milk or formula the baby eats; how often the infant normally eats; how much is normally consumed, special instructions for preparing and/or heating the bottles; whether to discard or return unfinished bottles; and any special instructions for communicating with parents about feedings, information about sensitivities or spitting up during or after feedings, burping the infant etc.

Comments:
A subsidy health and safety (SHSI) Complaint inspection was initiated on 9/20/2022 and concluded on 9/23/2022 in response to a complaint that was received by the department on 9/1/2022.
The inspector interviewed one staff on site on 9/21/2022 and reviewed documentation to include infant daily care sheets, related email messages, parent handbook, enrollment applications and signed agreements, written staff policies and procedures and child attendance documentation. Additional interviews were conducted on 9/22/2022 and two staff interviews were conducted remotely via video on 9/23/2022.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-790-820-H
Description: Based on interviews and review of documentation, the center staff did not ensure that formula or breast milk does not remain unrefrigerated for more than 2 hours and that it is not reheated.

Evidence:
1. A complaint was received on September 1, 2022, alleging that a 4 month old infant in care had been fed and re-fed or re-offered the same bottle over a period of more than 4 hours.
2. Interviews with the infant's parent revealed that 4 bottles of milk had been sent to the center for the infant on 8/26/2022 and that 3 of the 4 bottles were returned to the parents at the end of the day containing all of the milk placed in them that morning.
3. The infant's written Daily Care Report dated 8/26/2022, documented that infant #1 was fed a bottle at 8:20 a.m. and drank 2 ounces, was fed a bottle at 10:35 and drank 3 ounces and was offered a bottle at 1:00 p.m. which infant #1 refused.
4. Staff #3, who was the primary caregiver for infant #1 when he was in care at the center, was interviewed on 9/23/2022.
5. Staff #3 confirmed that a bottle of milk had been warmed and fed to infant #1 at 8:20 a.m., and then refrigerated and then the same bottle was reheated before being fed to infant #1 again at 10:35 a.m.
6. Staff #1 stated that she thought a milk bottle could be fed to an infant twice before disposing of the milk or returning it to the family, so she opened a new bottle at 1:00 and warmed it for infant #1. She stated that infant #1 refused the bottle offered at 1:00 p.m. and that she returned all unconsumed milk bottles to the family at the end of the day.

Plan of Correction: The following written plan of correction was received by the inspector on 10/12/2022:
"A portion of the Subsidy Program Vendor Requirements for Child Day Center, Section 8VAC20-790-820 'Special Feeding Needs' was published in the infant room, we also proceeded to provide this information to all the teachers of our center, all of them with the purpose of continuing to guarantee good care for our children."
