An unannounced monitoring inspection was conducted today, 10/28/15. Violations from the previous inspection have been corrected. A violation notice was completed regarding the violation found today. The children were napping during today's inspection. There are seven in care totaling 16 points with two caregivers. Should you have any questions please do not hesitate to call. Inspection began: 12:10 pm and ended approximately 1:45 pm.
Standard #: 22VAC40-111-100-B Description: Based on review of children's records a file was missing a physical examination. Evidence: 1. Child # began care on 2/3/14 and there is no documentation of a physical in the file. Plan of Correction: The provider will ensure that the physical is in the file and will review all other files to make sure all required information is on file.
