Technical Assistance:

Conducted an unannounced monitoring inspection. Observed 2 children = 5 points with the Provider. Ratios and supervision were in compliance. Children were waking from naps, playing quietly inside with blocks, puzzles, toy cars, listening to music, reading books, and eating snack which included: crackers, grapes, and milk. Earlier in the day the Provider + children went for a walk around the neighborhood. The Provider was talking with the children and assisting them as needed with tasks. The daycare areas of the home were found to be clean, safeguarded appropriately, and supplied sufficiently with toys and equipment for the children. No medications reported to be on-site. Emergency supplies were in compliance. First Aid/CPR certifications current. Annual training requirements met. Required documents posted. Record keeping violations found. Description: Based on records reviewed, it was determined that the emergency preparedness plan has not been reviewed on an annual basis. Evidence: The documented date that it was last reviewed by the Provider was in September 2014.

Plan of Correction: Reviewed Emergency Preparedness Plan and signed

Standard #: 22VAC40-111-60-B
Description: Based on 2 children's records reviewed, it was determined that 1 record did not include required information. Evidence: Child #10 - 1 of 2 parents work information to include address + telephone number, and medical insurance information to include policy number.

Plan of Correction: Received all needed information from parents.
