54.1-3408 Provider must be MAT certified to administer prescription medication.

Today an unannounced monitoring visit was conducted beginning at 11:45am. There were 11 children present today (28 points) with the Provider and assistant. Children were observed playing outside on the playground, washing hands, eating lunch, and napping during this inspection. Eleven children's records and 6 adult records were reviewed today. There are currently no medications on-site at this time for children. This inspection, including the exit interview, ended at 1:30pm. Should you have any questions, please call.

Standard #: 22VAC40-111-120-B
Description: Based on record review, one caregiver record (#7) is missing documentation that two references were checked prior to their first day of employment. Evidence: Caregiver #7 began employment on 9/8/15. There is no documentation of reference checks in their file.

Plan of Correction: I will document the reference checks and place them in their file. I did do them verbally on the telephone but did not record them.

Standard #: 22VAC40-111-60-B
Description: Based on record review, 2 children's records (#4 and #7) are missing complete required documentation in their files. Evidence: Both children's records #4 and 7 are missing addresses for both emergency contacts listed. Child #4, who has a start date of 5/11/15 has no documentation of a physical examination or updated immunization records in their file.

Plan of Correction: I will get this information from the parents immediately.

Standard #: 22VAC40-111-180-A
Description: Based on record review, one adult record (#2) is missing documentation of an updated TB test within 2 years of the most recent results. Evidence: The most recent documentation of a TB test for adult #2 is dated 3/2013.

Plan of Correction: We will make sure this is updated immediately and that all TB records are updated every two years.

Standard #: 22VAC40-111-200-A
Description: Based on record review, one caregiver record (#7) is missing documentation that they received orientation by the end of their first week of assuming job responsibilities. Evidence: Caregiver #7 began working on 9/8/15. The orientation checklist in their record is not filled out or signed documenting that they received the training.

Plan of Correction: We did do this training verbally when the caregiver started. We will go over the checklist again together and document that they have received the training.
