22VAC40-191 Background Checks for Child Welfare Agencies
An unannounced monitoring inspection was conducted today. The provider and two assistant caregivers were caring for 9 children, totaling 24 points. The children were observed playing with toys, assembling puzzles, coloring pictures, and interacting with each caregiver. The child care area is very clean and well-equipped with a variety of age-appropriate toys and materials. This inspection was completed between approximately 9:45 am and 11:00 am. Standard #: 22VAC40-111-760-A-1 Description: Based on a review of emergency supplies, it was determined that the provider does not have all required components inside the first aid kit. Evidence: The first aid kit does not include a second triangular bandage. Plan of Correction: I will buy a triangular bandage or make one out of fabric.
Standard #: 22VAC40-111-920 Description: Based on observations, it was determined that the current menu was not dated as required. Evidence: The menu for the current one-week period was not labeled with the dates of the current week. Plan of Correction: I will include the date on the menu each week.
Standard #: 22VAC40-111-960-E Description: Based on observations, it was determined that two infant bottles of breast milk were not labeled as required. Evidence: Two bottles of breast milk were not labeled with the child's name. Plan of Correction: The bottles will be labeled every day.
Standard #: 22VAC40-111-120-B Description: Based on a review of caregiver records, it was determined that the files for both assistant caregivers are not complete. Evidence: The files for assistant caregiver #1 and #2 do not include the documentation of a second reference check. Plan of Correction: I will document the completed reference check for each of them.
Standard #: 22VAC40-111-60-B Description: Based on a review of children's records, it was determined that three records were not complete. Evidence: 1. Child record #2 does not include the information for a second emergency contact person. 2. Child record #3 does not include documentation to show that the parent has conducted an annual review of the child's contact information. 3. Child record #4 does not include: documentation to show that the parent has conducted an annual review of the child's contact information; a signed "Declaration of Liability Insurance" document. Plan of Correction: I've already asked each parent for the missing information. I will ask each parent again.
Standard #: 22VAC40-111-530-F Description: Based on observations, it was determined that soft bedding was used for one infant while asleep in his crib. Evidence: A 4 month old infant was observed sleeping in his crib. The provider had swaddled the infant in a soft blanket. Plan of Correction: I will ask the parent to provide a safe sleep sack for the baby.
Standard #: 22VAC40-191-40-D-5-a Description: Based on a review of background check records, it was determined that the provider has not completed all required background checks. Evidence: One family member who resides in the home has become of age that a Child Protective Services (CPS) registry check is now required. The provider did not submit CPS registry check request for this family member. Plan of Correction: I will send the CPS request this week.
