Comments:
An unannounced renewal inspection was done today November 20, 2015 beginning at 10: 00 AM and ending at 2:00 PM. There were four children in care with the provider and assistant. Points were in compliance. The children were observed during part of their circle time. They also worked on a craft, played outside and practiced for their Thanksgiving program. Lunch and nap were also observed. Children, household and assistants files were reviewed. There are no medications given at the home and the provider continues with the USDA program. The provider and inspector discussed the violations and the corrective action. If you have any further questions, please call the inspector at 540-309-2835.

Violations:
Standard #: 22VAC40-111-90-C
Description: Based on observation the provider did not obtain the immunization updates for children under the age of two. Evidence: 1, During the inspection the LI reviewed four children's files and child # 4 did not have updated immunization verification. The child is age 23 months and the last updated verification is dated 1/8/15.

Plan of Correction: The provider will ask the parent to obtain the needed immunization verification.

Standard #: 22VAC40-111-170-B
Description: Based on observation the provider did not have all the required information for the TB test of an assistant. Evidence: 1. During the inspection the LI reviewed the files of assistant # 1 and the TB test is current however it is not signed by a physican or designee of the physician's office.

Plan of Correction: The provider will have the assistant obtain the needed information for the TB test.

Standard #: 22VAC40-111-240-A
Description: Based on observation the provider did not maintain the household area in a safe manner. Evidence: 1. During the inspection the LI observed the tile in the kitchen around the drain is loose and could potentially be a trip hazard.

Plan of Correction: The provider will repair the tile.

Standard #: 22VAC40-191-40-D-5-a
Description: Based on observation the provider did not obtain the required background checks for all household members. Evidence: 1. During the inspection the LI reviewed the files of household members and household members # 3 & 4 turned age fourteen since the last inspection. The provider has not sent for the child protective service checks.

Plan of Correction: The provider indicated she will send off the forms for the household members.
