Model forms given to Provider: Caregiver reference form. Discussed with provider: visitors to the day care.

Comments:
An unannounced Renewal study was conducted today. There are 5 children present (13 points) with 2 adults. 1 adult and 2 children's files were reviewed. See Violation notice for missing information. Children were observed resting quietly, being fed, being diapered, using the bathroom and washing hands. Emergency escape drills, menu, rest supplies, and the fridge temperature are all in compliance. Due to case load adjustments, your next inspection will be with a different inspector. Standard #: 22VAC40-111-960-E
Description: Based upon direct observation, bottles are not labeled and dated.

Plan of Correction: I will label the bottles as they arrive.

Standard #: 22VAC40-111-120-B
Description: Based upon a review of files, one adult file is incomplete. Evidence: The file for the Assistant does not contain documentation of 2 completed references prior to starting work.

Plan of Correction: The references will be on file by Monday.

Standard #: 22VAC40-111-320-A
Description: Based upon direct observation, small appliances were plugged in, that were not in use. Evidence; A Toaster and coffee maker where plugged in, in the upstairs kitchen area.

Plan of Correction: Appliances were unplugged during the inspection. I will unplug the appliances when they are not in use.
