54.1-3408 Provider must be MAT certified to administer prescription medication.

An unannounced renewal study was conducted from 10:00 am - 11:00 am. There were 4 children in care with the provider and one assistant ages 8 months, 14 months, 2 years, and 2 years. The children were involved in free play. Lunch served today was hot dogs, bananas, peas, bread, and milk. The provider record and one assistant records were reviewed and in compliance. Four children's records were reviewed and in compliance. The first aid kit, flashlight, and weather band radio were reviewed and in compliance. The provider is current in CPR and First Aid training. Standard #: 22VAC40-111-100-A
Description: Based on review of 4 children's records it was determined that not all children had documentation of a physical within 30 days of enrollment. Evidence: Child A with a start date of 11/01/2015 did not have a physical on file.

Plan of Correction: The provider will request an up-to-date physical for child A.

Standard #: 22VAC40-111-240-A
Description: Based on observation of the family day home it was determined that not all areas were maintained in a clean condition. Evidence: The exersaucer had dried food on the surface of the exersaucer.

Plan of Correction: The provider cleaned the exersaucer during the inspection.
