63.2 Liability Insurance Disclosure
Today an unannounced monitoring visit was conducted with the Provider and Assistant for between 9:45-11:30 there were 3 children present with a total of 12 points within the supervision guidelines. The children were observed having morning free play, story-time, feeding and in transition for lunch and nap time. A complete inspection of the physical plant, children and provider records, fire drill log, attendance record, and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-960-E Description: Based on observation of the children in care and bottles present, three bottles were found without the date. Evidence: Two infants were present however three bottles did not include the date. Plan of Correction: Provider stated that she will inform the parent to include the date and or will add tape daily with the date on each bottle.
Standard #: 22VAC40-111-60-B Description: Based on record review of three children's files, two files did not include documentation of the parents work address, and two designated emergency contact persons. Evidence: Child #2's file does not have the 2 emergency contacts maintained in the event the parents cannot be reached (Repeat violation). Child #3's file does not have the father's work address. Plan of Correction: Provider stated that she will get the information today from the parents.
Standard #: 22VAC40-111-690-D Description: Based on observation of the diapering area, the diaper changing surface was found ripped/torn and no longer nonabsorbent. Evidence: The diaper changing pad was found ripped/torn. Plan of Correction: Provider stated that she will replace it this weekend.
