Plan of Correction: I will ask the parents to complete the files. I will review all files.

Description: Based upon direct observation, a small appliance was plugged in, while not in use. Evidence: A hair dryer was plugged in, in an upstairs bathroom while not in use.

Plan of Correction: I will unplug the dryer when not in use.
