22VAC40-191 Background Checks for Child Welfare Agencies
Standard #: 22VAC40-111-100-A Description: Based on review of eight children's records it was determined that the provider failed to obtain a physical for each child in care before the end of the first thirty days of enrollment. Evidence: Child A with a start date of 5/25/2015 did not have documentation of a physical on file. This is a repeat violation. Plan of Correction: The provider will request an up to date copy of a physical for child A.
Standard #: 22VAC40-111-240-A Description: Based on observation of the kitchen in the day care area it was determined that not all areas of the family day home were in a safe and operable condition. Evidence: The LI (Licensing Inspector) observed one of the kitchen drawer handles was broken allowing a protrusion to be present. Plan of Correction: The provider removed the drawer handle during the inspection.
Standard #: 22VAC40-111-540-B Description: Based on observation of the family day home it was determined that walkers were used in the family day home. Evidence: The LI (Licensing Inspector) observed an infant walker in the day care area of the family day home. Plan of Correction: The provider removed the infant walker during the inspection.
Standard #: 22VAC40-191-40-D-1-c Description: Based on record review of one household member it was determined that not all background checks were repeated every three years. Evidence: The most recent Central Registry Check for Household Member A was dated 7/26/2012. Plan of Correction: The provider will submit a new central registry check for household member A.
