63.2 Liability Insurance Disclosure
An unannounced "other " inspection was conducted today by the licensing inspector. The licensee was present with 3 family children and 2 other children were in care during the inspection. Children were playing monopoly, playing with wrestling figures and using the computer. A sample of children's records were reviewed today. There were violations cited today that had been previously cited at the initial inspection. Please be aware that violations are expected to be corrected within the time frame indicated on the violation notice. snack today- apple juice, crackers lunch menu - chicken patty, oranges, green beans, milk evacuation drill - 1/4/16 shelter in place - 1/6/16 approximate time on site: 3:20 pm - 4:45 pm
Standard #: 22VAC40-111-100-B Description: Based on review of children's records, the licensee did not obtain documentation of a physical exam. Evidence: Child #2 did not have documentation of a physical examination and the child has been in care more than 30 days. Plan of Correction: I will ask the parents to provide the information as soon as possible.
Standard #: 22VAC40-111-50-A Description: Based on observation, the licensee did not have a daily attendance sheet. Evidence: There was no written record of children in attendance at the time of the inspection. Plan of Correction: Corrected on site during the inspection.
Standard #: 22VAC40-111-60-B Description: Based on review of children's records, the licensee did not have a record for each child in care.
Evidence: Child #1 - There was no file for this child who was in care at the time of the inspection.
Plan of Correction: The child is a drop in and does not attend every day. I will contact the parent and have the information provided as soon as possible.
Standard #: 22VAC40-111-90-C Description: Based on review of children's records, the licensee did not obtain documentation of immunizations. Evidence: Child #2 did not have documentation of immunizations and has been in care more than 4 months. Plan of Correction: I will contact the parents and have them provide the information as soon as possible.
Standard #: 22VAC40-111-240-A Description: Based on observation, the facility is not being maintained in a safe condition.
Evidence: There is a hole in the chain link fence of the side of the yard that is large enough that a child could slip through to the next yard. The crawl space door/coverings on the outside of the house (backyard) are missing.
Plan of Correction: I will go to the store this weekend and get the materials need for the repair. The person doing the repair work will be done by 2/1/16. I will cover the crawl space area before then to keep children out and safe.
Standard #: 22VAC40-111-440-B Description: Based on observation, the protective barriers/guard rails on the front porch do not prevent a child from climbing over, through or being entrapped.
Evidence: The space between the pickets varies from 4 1/2 in. to 4 3/4 in. There is a circular area where the width spacing is approximately 7 in.
Plan of Correction: I will get the materials and have the man fix it by Feb 1.
