Comments:
A code compliance inspection was conducted October 18, 2022. The licensing inspector was onsite from approximately 2:30pm until 3:45pm. There were eight children present, ranging in ages from two to four years, with one staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of five children's records and two staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.031-B-5
Description: Based on observation, the center did not ensure all areas of the premises accessible to children are free of obvious injury hazards, including providing and maintaining sand or other cushioning material under playground equipment.
Evidence:
The door in the hallway leading into the kitchen was broken and hanging off the hinges.

Plan of Correction: We have purchased lots of carbon monoxide detectors for all of our centers. We were waiting until our painting was completed . We are installing them in all of our centers now. See attached invoice

Description: Based on observation and interview, the child day program, which was built prior to 2015, was not equipped with at least one carbon monoxide detector.
Evidence: A member of management stated that they have not yet installed a carbon monoxide detector.

Plan of Correction: The door to our kitchen has been ordered its been on backorder at local store due to the pandemic items has taken longer to receive . The door is due to be installed as soon as received, Children are never allowed in the kitchen.,
