Technical Assistance:
**Discussed with the provider's daughter that the background checks that were returned to the home, because of incomplete information, are sent back to be processed ASAP.

Comments:
An unannounced renewal inspection was conducted today from 10:30am-11:46am. There were 6 childcare children (20 points) directly supervised by 1 staff, but a second staff member arrived about 15 minutes after the licensing inspector began the visit. The home, 2 staff records, 3 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely and napping. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in this report. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. Violations:
Standard #: 22VAC40-111-110-B
Description: Based on review, the provider failed to ensure that each child's immunization record is signed by a licensed physician, the physician's designee or an official of a local health department. Evidence: Reviewed 3 children's records and found that CH3 did not have documentation of a signed immunization record.

Plan of Correction: Mom had the doctor fax us immunization with doctors seal on it and sign.

Standard #: 22VAC40-111-60-B
Description: Based on review, the provider failed to ensure that children's records are maintained with the required information. Evidence: 1) CH1 did not have documentation of parents work information. 2) CH1 only had 1 emergency contact and CH2 did not have any documentation of an emergency contact.

Plan of Correction: We asked parent to provide work information. We asked parent to provide numbers in care of emergency.

Standard #: 22VAC40-111-90-A
Description: Based on review, the provider failed to ensure that before a child may attend the family day home, that documentation that the child has been adequately immunized is obtained. Evidence: Reviewed 3 children's records and found that CH1 did not have documentation of an immunization record.

Plan of Correction: Mom picked it up from the doctor and gave it to us upon pick up.

Standard #: 22VAC40-111-570-A
Description: Based on observation, the provider failed to ensure that a single caregiver does not exceed 16 points by using the point system. Evidence: Observed that there were 6 childcare children in care and 20 points for the single caregiver. The second caregiver had gone to a neighbor's home for an errand.

Plan of Correction: Provider will be in the day care and help out if there is more than 16 points using the point system.
