Technical Assistance:
The provider has reported an inability to open and print previous inspection documents. The Inspector will print and mail the violation notice and inspection summary for this inspection. The provider shall send documentation to the Inspector showing that the dog has received a rabies vaccination. The provider shall submit all background check requests prior to the arrival of one adult family member who will be moving into the home in March.

An unannounced full renewal inspection was conducted today. The provider and one assistant caregiver were caring for 8 children, totaling 26 points. The children were observed taking their morning nap, playing with toys, and preparing for lunch. The children were to be served spaghetti, fish sticks, apples, bread, and milk for lunch today. Standard #: 22VAC40-111-50-A
Description: Based on a review of administrative documents, it was determined that the provider did not have an accurate record of children in attendance today. Evidence: Upon arrival, there were 8 children present and in care. However, the documented attendance indicated a total of only 6 children present and in care.

Plan of Correction: The attendance was corrected immediately during the inspection.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that one record was not complete. Evidence: Child record #4 does not include documentation to show that the provider has viewed the child's original proof of ID.

Plan of Correction: The parent showed me a copy of the proof of birth letter. I will ask the parent to show me the original letter or the child's original birth certificate.

Standard #: 22VAC40-111-180-A
Description: Based on a review of caregiver health records, it was determined that the provider has not obtained a repeat TB Health Screening within the past year. Evidence: The provider's TB Health Screening expired on 2/10/16.

Plan of Correction: I will get a new TB screening done.

Standard #: 22VAC40-111-200-A
Description: Based on a review of caregiver training records, it was determined that the provider did not conduct orientation training as required. Evidence: The current assistant caregiver began employment on 2/16/16. The provider has not conducted and documented orientation training for her.

Plan of Correction: I trained her on the emergency preparedness portion of the orientation training. I will complete the rest of the training with her and then put the documentation in her file.

Standard #: 22VAC40-111-240-A
Description: Based on observations, it was determined that one area outside of the home has not been maintained in a safe condition. Evidence: The provider has placed several large pieces of children's play equipment in front of the door that leads from the basement child care area to the outside. These items block the stairway where the children would evacuate in the event of an emergency.

Plan of Correction: I will move the toys and play pieces today.

Standard #: 22VAC40-111-460-C
Description: Based on a review of administrative records, it was determined that one of the provider's dogs does not have a current rabies vaccination. Evidence: The rabies vaccination for one of the provider's dogs expired on 1/11/16.

Plan of Correction: I thought he got a 3 year rabies vaccination. I will take him to the vet for a new rabies shot.

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of caregiver background checks, it was determined that the provider did not submit one background check request within 7 days of the caregiver's date of hire. Evidence: The current assistant caregiver began employment on 2/16/16. As of the date of this inspection on 2/25/16, the provider had not submitted the Child Protective Services (CPS) registry search request for the assistant caregiver.

Plan of Correction: I will submit it today.
