Comments:
Today an unannounced monitoring visit was conducted beginning at 12pm. There were two children (5 points) present today with the Provider. Children were observed eating lunch and playing with stickers during the inspection. Adult and children's records were reviewed today. There are no medications on-site for children enrolled. This inspection, including the exit interview, ended at 1pm. Should you have any questions, please call.

Standard #: 22VAC40-111-170-A
Description: Based on record review, a new adult household member (#4) has no documentation of a TB test.

Plan of Correction: We will make sure this person gets a TB test immediately.

Standard #: 22VAC40-111-180-A
Description: Based on record review and discussion, one adult record (#1) has no documentation of an updated TB test dated within 2 years of the most recent results. Evidence: The last TB test results for adult #1 is dated January, 2014. There is no updated record.

Plan of Correction: We will get this renewed immediately and renew it every two years.

Standard #: 22VAC40-191-40-D-1-a
Description: Based on record review and discussion ,one adult household member (#4) is missing complete background checks in their record. Evidence: Adult #1 is missing documentation of a completed sworn disclosure statement.

Plan of Correction: We will make sure this is filled out and dated immediately.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on record review, one adult record (#1) is missing documentation of a repeat background check updated within 3 years of the most recent results. Evidence: Adult #1 does not have an updated search of the central registry with results dated within 3 years of the last results. Most recent results are dated February, 2012.

Plan of Correction: We will submit this immediately and renew them every three years.
