Comments:
An unannounced monitoring inspection was conducted on March 14, 2016 at 9:45 a. m. and concluded at 11:45 a. m. The inspector met with the provider and some but not all Family Day Home standards were reviewed. There were 12 children present with one of those children residing in the home, and points equaling 19 with two caregivers. Upon the inspector's arrival the children were observed having circle time where they talked about the month, days of the week, day of the week, year, counting, reciting nursery rhymes and many other activities. Later they enjoyed the snack that each child brought in. The family day home is set-up with a variety of materials and equipment for children. Three children records were reviewed. Three household member records were reviewed. Conversations were held with two children and two caregivers. No medication being administered today/Provider completed MAT (medication administration trained) on 7/19/14. The last monthly evacuation drill was dated 2/24/2016

Description: Based on review of household member records one person did not have an updated sworn disclosure statement. Evidence: On 3/14/16, after review of all household member records, #1 had a sworn disclosure statement dated 7/8/12.

Plan of Correction: I will get this updated by the end of this week.
