A renewal inspection was conducted today. Observed 4 children (11 points) directly supervised by 1 caregiver. Ratio and supervision were in compliance. A sample of 2 children's records, 1 caregiver record, and 1 household member record were reviewed. The physical plant, emergency supplies and procedures were also reviewed. No medications were reported to be on site today. The children were engaged in various activities, to include: free play, coloring, hand washing, potty training, and diapering. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Standard #: 22VAC40-111-60-B Description: Based upon documentation reviewed, a child's record was incomplete. Evidence: One of the two children's records reviewed did not contain the policy number of the child's medical insurance(Child A; Date of enrollment (DOE) 02/29/2016). Plan of Correction: I will as the mother again for the information.
Standard #: 22VAC40-111-90-A Description: Based upon documentation reviewed, the provider did not obtain documentation that a child has been adequately immunized before the child attended the family day home. Evidence: 1) One of the two children's records reviewed did not contain immunization records (Child A; DOE 02/29/2016). 2) Child A was in attendance during today's inspection. Plan of Correction: I will ask the mother again for the information.
Standard #: 22VAC40-111-180-A Description: Based upon documentation reviewed, the provider did not obtain for each adult household member a current Report of Tuberculosis (TB) Screening form very two years from the date of the first screening. Evidence: Household member #1 did not have a current TB form, last TB form was dated 03/05/2014. Plan of Correction: A new TB test will be obtained.
