54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was conducted from 9:30 a.m.-10:30 a.m. with the provider and assistant. Eight daycare children, ages 6 months, 13 months, 18 months, 2 years, 2 years, 3 years, 4 years, and 5 years and three resident children, ages 8 months, 4 years, and 6 years were in the care of two caregivers for a total of 27 points. Children were observed having an Easter egg hunt, eating morning snack, listening to a story, building with blocks, and playing with the dollhouse. Morning snack today was rice cakes and 100% apple juice. Both the provider and assistant are current in first aid and CPR. The last emergency escape drill was 2/17/16. One staff record was reviewed and in compliance. Two children's records were reviewed. Standard #: 22VAC40-111-100-A Description: Based on review of two children's records, the provider did not have documentation of a physical in each child's file prior to or within 30 days of first attendance. Evidence: Child A's (enrolled 8/24/15) record did not have documentation of a physical. Plan of Correction: The provider will notify the parent of this at pick-up.
