An exit interview was conducted with the provider to discuss the results of today's inspection.
An unannounced monitoring inspection was conducted today, April 27, 2016 to determine the provider's compliance with licensing standards. In attendance was one child totaling 3 points being supervised by the provider. The child was observed interacting with the provider and playing with lego blocks and toys. Today's lunch consisted of grilled cheese sandwich, tomato soup, mixed fruit, and milk. Children's books, toys, and equipment were inspected and found to be age appropriate and in operable condition. The children?s restroom was inspected with the required supplies present. All rooms of the home used by children were inspected.
The family day home had the required postings displayed on the bulletin board upon entering the home. The first aid kit, fire extinguisher, and emergency supplies were inspected. One child's record was inspected with one violation found. The following was also inspected: nutrition requirements, outdoor play area, parent agreements, and other pertinent information. See attached notice for violations found on this day.

Standard #: 22VAC40-111-30-A Description: Based on the visual inspection of the family day home, the provider did not comply with the terms stipulated on the current conditional license issued. Evidence: Upon arrival to the family day home to conduct an unannounced monitoring inspection, this inspector observed three canines at the storm door with a child. Upon further investigation, the child at the door was enrolled receiving day care services. Based on the stipulations written on the license "Children will not have access to the canines in care," the provider was not in compliance with the terms of the current license issued. Plan of Correction: The provider corrected the issue on site and informed children in care will no longer have access to canines as it was a misunderstanding.
Standard #: 22VAC40-111-60-B Description: Based on the inspection of children records, a required component was not listed. Evidence: During the inspection of one child's record, it was observed that the medical policy number was not listed. Plan of Correction: Per provider: I will obtain the information from the parent today at pickup time.
