Technical Assistance:
A supplemental page was provided. It is my understanding that you are not using the upper section of the large play structure and the yellow slide on the playground because those two pieces of equipment do not meet the licensing standards.

Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted on May 13, 2016. The on-site inspection was conducted from 9:55 a.m. to 12:25 p.m. On this date, there were eight (8) children in the care of the provider for sixteen (16) points. There were a total of thirteen (13) children enrolled in the program. There was one (1) child's record reviewed and the records of the provider and one (1) assistant were checked for updates. The provider and three (3) children were interviewed. There were no medications being administered at the time of the inspection, which is the policy of the provider. The violations from the previous inspection were reviewed for compliance. The exceptions to compliance were noted on the violation notice and the risk ratings for the violations are noted on the supplemental page. Lunch was observed to meet USDA guidelines. Outdoor play was not observed due to rain. The letter of the week was Y,y and there was an ample supply of age appropriate toys and equipment. Choice time, show and tell, hand washing and bathroom procedures, diaper changing procedures, infant feeding, lunch, and nap time were observed during the visit.

Violations:
Standard #: 22VAC40-111-920
Description: Based on observation and verification from the provider on 05/13/2016, the provider failed to post the week's menu of food being served. Evidence: 1. The menu of foods being served by the provider for the week of May 9th through May 13 was not posted in an area accessible to the parents. 2. The provider confirmed the menu was not posted.

Plan of Correction: During the inspection, the provider posted the menu. In the future, she will ensure to post the menu each week.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on review of caregivers' records and verification from the provider on 05/13/2016, the provider failed to obtain updated background checks within the last three years. Evidence: 1. There was no updated criminal history record check for Caregiver #1. The date of the last criminal history record check was 04/04/2013. The provider sent the request for the record check on 04/01/2016. 2. There was no updated search of the central registry for Caregiver #1. The date of the last search of the central registry was 04/19/2013. The provider sent the request for the search on 04/05/2016.

Plan of Correction: The provider will ensure that in the future she will send the requests to update background checks early enough so they can be completed and returned before three years since the last ones were completed.
