Comments:
An unannounced renewal inspection was conducted today from 10:05 - 12:00. There were 7 children in care = 24 points with two caregivers. This includes one related child. The children participated in Montessori activities, played with trains, practiced writing, had circle time and sang songs. They ate lunch which is brought from home. The exit interview was conducted with the licensee. The violation notice will be e mailed for completion within 10 days.

Standard #: 22VAC40-111-760-A-1
Description: Based on a review of items in the first Aid kit, the licensee did not have all of the required items. Evidence - There were no triangular bandages in the first aid kit.

Description: Based on a review of children's records, the licensee did not obtain all of the required information. Evidence - The record for child #1 did not contain the child's start date or the street address for the child's second emergency contact. The record for child #5 did not contain the child's start date.

Description: Based on a review of the children's records, the licensee does not provide all of the required written information to parents. Evidence - The parents do not receive the exact amount of tuition for their child, the daily schedule, a complete termination policy and the open door policy.
