Today an unannounced monitoring visit was conducted with the Provider. This visit took place in the morning between 9:15am-11:30 there were 3 children present with the Provider for a total of 10 points within the supervision guidelines. The children were observed having morning free play, playing with blocks, snack and in transition for lunch and nap time. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children records, fire drill log, and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-110-A Description: Based on a review of the children records, one record did not include documentation of a physical & immunization on the virginia health form. Evidence: Child #1 & #2's record did not have results of physical exam and immunization on a Virginia Health Form. Plan of Correction: Provider will notify the parent and give them a copy of the form to be completed.
Standard #: 22VAC40-111-60-B Description: Based on the children records, two records did not include documentation of two emergency contacts, parent's work address and phone number, medical insurance number, home address and phone number. Evidence: Child #1's record only had one emergency contact person name, address and phone number listed. Child #2's record did not include a child information form with the following information: two emergency contacts, parent's work address and phone number, medical insurance number, home address and phone number. Plan of Correction: Provider will inform parent and provide her with a copy of the child information form. Provider stated that Child #2 is new and parent will bring today.
Standard #: 22VAC40-111-340-C Description: Based on observation of the physical plant, the provider did not have any paper towels in the bathroom. Evidence: The provider did not have paper towels in the bathroom, hand towels were found hanging up on the wall. Plan of Correction: Provider will remove hand towels and make sure the children are using paper towels at all times.
