An unannounced other inspection was conducted on May 20, 2016. There were two children present during the inspection with the provider totaling 5 points. No medications are being administered in the family day home at this time. Two children's records and the provider's record were reviewed during the inspection. The outdoor play area is not being used at this time.
Standard #: 22VAC40-111-830-A Description: Based on review, the provider did not ensure that emergency evacuation procedures were practiced monthly. Evidence: During the inspection conducted on May 20, 2016, there was no documentation of an evacuation drill practiced in April. Plan of Correction: I will do an evacuation drill every month.
Standard #: 22VAC40-111-100-A Description: Based on record review, in one of two children's records reviewed, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: During the inspection conducted on May 20, 2016, there was no documentation of a physical examination in the record for Child #2 (start date as indicated in the record 9/17/15). Plan of Correction: An appointment has been scheduled for the child but I will talk to the parent about getting a copy.
Standard #: 22VAC40-111-60-B Description: Based on record review, in one of two childrern's reviewed, the provider did not ensure that a child's record had all required information. Evidence: During the inspection conducted on May 20, 2016, the record for Child #1 did not include documentation of a name, address and telephone number of one of two designated persons to contact in case of an emergency if the parent cannot be reached. Plan of Correction: I will talk to the parent today and have it changed.
Standard #: 22VAC40-191-40-D-1-c Description: Based on record review, the provider did not have a criminal history record check before three years since the date of the most recent criminal history record check report. Evidence: During the inspection conducted on May 20, 2016, the provider's criminal history record check report expired and was dated 5/8/13. Plan of Correction: The criminal record check has been mailed and awaiting return.
