The following inspection took place between the hours of 10:45 am through 12:15 pm. Today, I met with the Provider on site at the Family Day Home (FDH). At the start of this unannounced monitoring inspection there were a total of 8 children (27 points) with 2 caregivers. A total of 4 children's files and 2 staff/household member's files were reviewed. The children were engaged in the following: toy play, painting, snack, and diapering. Standard #: 22VAC40-111-100-A Description: Based on a review of children records, the provider has not obtained documentation of a physical examination prior to the child's attendance or within 30 days after the first day of attendance. Evidence: There is no documentation of a physical examination for child #3, first day of attendance 9/2014. Plan of Correction: The provider will request that the parent obtains a physical examination for the child.
Standard #: 22VAC40-111-120-A Description: Based on a review of staff records, the provider has not maintained a complete record for each caregiver. Evidence: There is no information for the current assistant; employment date, address, emergency contact information, and references. Plan of Correction: The provider will have the assistant complete the assistant provider record.
Standard #: 22VAC40-111-50-A Description: Based on a review of records and provider interview, the provider has not maintained daily attendance each day. Evidence: There was no documentation of children in attendance this date. The provider stated that she has not maintained attendance for some time now. Plan of Correction: The provider will maintain daily attendance.
Standard #: 22VAC40-111-60-A Description: Based on a review of children records, the provider did not maintain an up to date record for each child. Evidence: Child #3 record was last updated on 9/2014, not within the year. Plan of Correction: The provider will have the parent update the form.
Standard #: 22VAC40-111-60-B Description: Based on a review of children records, the records are not complete. Evidence: Child #4 record was missing a complete emergency contact address. (repeat) Child #3 record was missing the medical insurance policy #. Plan of Correction: The provider will have the parent complete the addresses and the medical insurance policy #.
Standard #: 22VAC40-111-90-C Description: Based on a review of children records, the provider has not obtained documentation of additional immunization once every 6 months for children under the age of two years. Evidence: The Immunization record for Child # 3, date of birth 6/23/2014 was last updated on 12/7/2014. Plan of Correction: The provider will request updated immunization record from the parent.
Standard #: 22VAC40-111-170-A Description: Based on a review of caregiver records, the provider did not obtain from each caregiver at the time of hire a current report of Tuberculosis screening. Evidence: Caregiver #1 has been working since 2/2016, she does not have documentation of a Tuberculosis screening. Plan of Correction: The caregiver will provide a copy of the TB screening.
Standard #: 22VAC40-111-200-C Description: Based on a review of caregiver records, the provider has not obtained documentation that the caregiver received orientation by the end of their first week of assuming job responsibility. Evidence: There was no documentation of staff #1 receiving orientation. Staff assumed job responsibility on 2/2016. Plan of Correction: The provider will have the caregiver complete and sign off on the Orientation Training form.
Standard #: 22VAC40-111-530-A Description: Based on observation, the provider is not maintaining a top and bottom cover for each crib. Evidence: When the licensing inspector arrived, infants were observed sleeping in four different cribs. Three out of four of the cribs did not have a bottom sheet. Plan of Correction: The provider will maintain a bottom fitted sheet for each crib.
