6 children present with 2 caregivers totalling 15 points during this monitoring inspection. The children were observed eating lunch , today they had chicken nuggets, spinach, rolls and apples. The children are reading books and getting ready for nap. Lots of interaction between children and caregivers. The provider has lots of toys for the children to explore. Standard #: 22VAC40-111-100-A Description: One child out of 4 are missing a physical. Evidence Based on the review of 4 children's files , one child, child A is missing a physical. Plan of Correction: I will ask the parent for the information.
Standard #: 22VAC40-111-120-B Description: Provider failed to get a second reference for the assistant. Evidence: Based on the review of two caregiver files , the assistant is missing a second reference. Plan of Correction: I will make sure I get the second reference.
