54.1-3408 Provider must be MAT certified to administer prescription medication.
An announced initial inspection was conducted today from 9:00am-9:45am. There were 2 children (5 points) directly supervised by the provider. The home, the provider's record, 5 children?s records, 3 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-920 Description: Based on provider statement, the menu for the current one-week period was not posted in the home. Evidence: Documentation of the current week's menu was not observed posted in the home. The provider stated that parents are verbally informed of the daily menu. Plan of Correction: I will complete today.
Standard #: 22VAC40-111-100-A Description: Based on record review, the provider did not obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. Evidence: Documentation of a physical examination was not observed documented for Child 1(enrolled March 21, 2016), Child 3 (enrolled August 23, 2013), Child 4(enrolled August 17, 2015) and Child 5(enrolled January 18, 2016). Plan of Correction: I will ask today.
Standard #: 22VAC40-111-60-B Description: Based on record review, children's records were incomplete. Evidence: 1. The father's work information was not observed documented for Child 1, Child 4, and Child 5. 2. The doctor's information was not observed documented for Child 1, Child 4 and Child 5. 3. A second emergency contact person was not observed documented for Child 3 and Child 4. 4. The medical insurance policy number was not observed documented for Child 1 and Child 2. 5. The parent's signed acknowledgement of the receipt of the Liability Insurance Declaration form was not observed documented for Child 1, Child 2, Child 3, Child 4 and Child 5. 6. The parent's signed acknowledgement of the receipt of the Policy for the Administration of Medication form was not observed documented for Child 2 and Child 3. Plan of Correction: I will talk to the parents today.
