Comments:
An unannounced complaint inspection was conducted today regarding allegations of nutrition, television and children's linens. The home was inspected, interviews conducted and records reviewed. A preponderance of evidence does not support the allegations, thus the complaint is deemed not valid, however, non-complaint related violations were cited and are identified in the Violation Notice. Complaint related: No
Description: Based on record review, emergency evacuation procedures were not practiced monthly. Evidence: An emergency evacuation drill was not observed documented for May 2016 and June 2016. The last documented drill was completed on April 9, 2016.

Plan of Correction: I did it. I will find the form today.

Standard #: 22VAC40-111-180-A
Complaint related: No
Description: Based on record review, the provider did not obtain documentation of a current Report of Tuberculosis Screening every two years from the date of the previous screening for a caregiver and household member. Evidence: 1. The TB Screening documentation observed for Caregiver C was last completed on June 27, 2014 and expired on June 27, 2016. 2. The TB Screening documentation observed for Household Member G was last completed on September 12, 2013 and expired on September 12, 2015.

Plan of Correction: They will get it completed.

Complaint related: Yes
Description: Based on record review, the provider did not obtain a current sworn statement or affirmation before three years since the dates of the previous statement for household members. Evidence: 1. The sworn statement or affirmation observed for Household Member D was last completed on July 1, 2013 and expired on July 1, 2016. 2. The sworn statement or affirmation for Household Member F was not observed in the record, however, the sworn statement was observed during the March 22, 2016 inspection and was completed on February 22, 2016.

Plan of Correction: I have it. I will find it and put it in their file.
