An unannounced mandated monitoring inspection was conducted today. The inspector arrived at 2:10 PM and departed at 3:30 PM. Four children, totaling 16 points, were in care with two caregivers. Napping children, activities and afternoon snack service was observed. Although the children were not outside during the inspection, the backyard play area was inspected. Four children's records and two caregiver records were reviewed. An exit meeting was conducted with the provider, during which the plan of correction for the violation cited was obtained from the provider. Please contact the inspector if there are any questions.
Standard #: 22VAC40-111-80-A Description: Based upon review of four children's records and provider interview, the provider has not ensured that proof of identification is obtained for newly enrolled children within seven business days of the child's first day of attendance. Evidence: 1. There is no documentation in the record of child 4, who began attending on 5/16/16, that proof of identification was obtained/verified. 2. The provider stated that the birth certificate for child 4 has not been provided for verification. Plan of Correction: The provider responded with the following: The parent will be told this evening that proof of identification must be provided when the child returns to care next Tuesday.
