An unannounced monitoring/renewal inspection was conducted today from 10:20am to 11:20am. There were 7 children (21 points) directly supervised by 2 staff. The home, 2 staff records, 1 child?s record, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely and preparing for lunch. Diapering procedures were also observed. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in this report. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. Description: Based on review, the provider failed to ensure that evacuation drills were documented on a monthly basis. Evidence: Reviewed the evacuation drill log and found that there was no drills documented since spring of 2016.

Description: Based on observation, the menu was not posted for the current one-week period. Evidence: Observed that the menu for the week was not posted.

Standard #: 22VAC40-111-50-A
Description: Based on review, the provider failed to ensure that a written record of children in attendance was documented each day. Evidence: Reviewed the attendance records and found that there was no documentation of attendance for the current day.

Plan of Correction: Will complete during nap time.

Standard #: 22VAC40-111-60-B
Description: Based on review, the provider failed to ensure that children's records were maintained with the required information. Evidence: Reviewed 1 child's record and found that they were missing the following... 1) There was no documentation of work information. 2) There was no address for the first emergency contact and there was not a second emergency contact listed. 3) There was no documentation of a medical insurance policy number. 4) There was no documentation of the parent's authorization of emergency medical care. 5) There was no documentation of information/agreement of parents, liability insurance, medication policy and emergency plan.

Plan of Correction: Parent is coming today to complete.
